Static cold storage at approximately 4°C remains a simple and widely used method for donor lung preservation; however, the optimal temperature for prolonged lung preservation remains uncertain. Controlled 10°C preservation has recently attracted renewed interest, but its feasibility with EP-TU lung preservation solution, an extracellular-type phosphate-buffered solution developed at Tohoku University, has not been clarified. This study evaluated the feasibility and early graft function of controlled 10°C preservation using EP-TU solution compared with conventional 4°C preservation in a rat orthotopic left lung transplantation model. Male Lewis rat donor lungs were flushed with EP-TU solution and preserved for 18 hours at either 4°C or 10°C (n = 4 per group). Orthotopic left lung transplantation was then performed using the cuff technique, followed by 2 hours of reperfusion. After reperfusion, the pulmonary venous oxygen partial pressure/fraction of inspired oxygen ratio was significantly higher in the 10°C group than in the 4°C group [153.5 mmHg (109.0–183.1) vs. 65.95 mmHg (27.0–83.4), p = 0.0286]. The wet-to-dry weight ratio tended to be lower in the 10°C group than in the 4°C group [7.240 (5.3988–7.9452) vs. 8.506 (7.5072–9.3987), p = 0.0571]. Histologic lung injury score, TUNEL-positive cell ratio, and inflammatory cytokine messenger RNA expression did not differ significantly between the groups. Controlled 10°C preservation using EP-TU solution was feasible and attenuated early impairment of graft oxygenation compared with conventional 4°C preservation after prolonged static storage, without significant differences in these injury markers. These findings support further investigation of EP-TU-based 10°C preservation as a strategy for safe and prolonged lung preservation.利益相反に関する開示
