感覚刺激によるガンマ同調(GENUS)の研究動向と課題

抄録

【目的】 本稿は、マサチューセッツ工科大学(MIT)によりアルツハイマー病(AD)に対する非薬理学的介入手法として提案された感覚刺激によるガンマ同調(Gamma Entrainment Using Sensory Stimuli:GENUS)に関する既報文献を、2026年上期段階までに公開済みの世界のGENUS研究について、動向と今後の課題を把握するためのナラティブレビューとして整理し、米国を中心とした研究動向、米国外における検証研究、および臨床・基盤研究上の主要な課題を概観する。
【方法】 著者が日常的に収集した文献を起点とし、PubMed、Google検索、J-STAGE、J-GLOBAL等を用いて関連文献をキーワード検索し、候補文献の見落とし防止のためにAI検索も補助的に用いた。文献の採択は、検索結果の書誌事項からオンラインで原典の抄録と本文を読んだ上で決定した。GENUSの直接介入研究、感覚刺激による40 Hz応答の基盤・機序研究、安全性・装置設計等を検討するための補助文献を含む56件を採択し、研究デザイン、対象、刺激条件、評価指標、結果および限界を整理した。なお、GENUSの技術上の重要課題に関する大量の研究事例のリストを取りまとめた総説あるいは文献学的な系統的レビュー文献も積極的に採択した。
【結果】 採択した56件は、本稿上の分類では、米国のGENUS研究に関連する文献19件、欧州14件、中国6件、台湾8件、日本4件、その他5件であった。前臨床研究では、40 Hz感覚刺激による刺激同期40 Hz応答、Aβ低下、神経保護、グリンパティック排出促進等の肯定的所見が報告された一方、内因性γ振動の同調やAβ低下を再現しない研究も認められた。ヒト研究ではSSVEP、ASSRその他の刺激同期40 Hz応答が多く報告されたが、認知機能などの改善が報告された研究は未だ小規模・探索的であった。短時間の身体活動直後に40 Hz光刺激を提示した探索的研究は1件確認されたが、運動療法とGENUSを継続的に併用した臨床研究は確認できなかった。
【考察】 GENUSの応答は、刺激周波数・波形・感覚モダリティ、脳状態、注意・行動状態、疾患段階および個人差によって変化する可能性が報告されている。外因性の刺激同期応答と内因性γ振動を区別し、安全性、刺激条件、計測条件および解析方法を標準化する必要がある。
【安全性上の論点】 ヒトを対象として周期的な光刺激を用いる研究では、光過敏性発作、頭痛、不快感等に配慮し、刺激条件の管理、事前スクリーニング、倫理審査および同意取得を要する。光刺激と音刺激の位相差については、GENUSにおける認知機能または安全性シグナルへの影響を直接検証した研究事例が乏しく、装置設計・評価上の未検証課題として位置づけるべきである。
