An 89-year-old female with hypertension and Alzheimer’s disease was referred to our hospital with postprandial nausea and vomiting. On this visit, she had eaten to her heart’s content. Chest radiography revealed a round, mass-like opacity without an air-fluid level. Thoracoabdominal computed tomography carried out the next day with hungry, revealed mesenteric herniation of a considerable part of the stomach and a part of the colon with mesenteric fat. Based on these findings, she was diagnosed with hiatal hernia (HH). The presence of an air-fluid level superimposed on the cardiac contour is a common chest radiographic finding of HH. In the present case, a mass-like opacity without an air-fluid level was observed on the chest radiography due to a full stomach. Therefore, clinicians should be aware that a mediastinal mass-like opacity without an air-fluid level on the chest radiography may indicate HH.
