作業療法とは何か——治療哲学としての再定義と境界領域専門性の確立

抄録

本稿の目的は、「作業療法とは何か」という問いに対して、従来の「治療法の名称」としての解釈を超
えた新たな枠組みを提示することである。作業療法士が自らの専門性を説明できないという問題は、作
業療法が特定の手技や介入の集合体ではなく、人間の作業可能性に介入する治療哲学であるという本質
から生じている。この哲学は、人ごとに実践が異なることを欠陥としてではなく固有の特性として説明
し、質的エビデンスによって正当化される専門性を持つ。また、作業療法は生医学的モデルが支配する
病院の内部に存在しながら、その論理には完全には属さない境界領域の専門家（Boundary-Domain
Professional）として位置づけられる。本稿は前稿（前田, 2026）が整体・徒手療法について提示した
「境界領域の専門性」という枠組みを継承し、それを作業療法に拡張する概念論文である。
